yeah there has, Ravens challenged the Browns fumbled and lost. I didn't count that as the first timeout though even though they lost one.

to make room so you're not throwing out of the end zone and can get a safety

nah, the runner is coming towards the line and will be at most 1-2 yards from the line of scrimmage. the passer is going back at least 5 yards and is just sitting there.
